PEAK: Kidney Care Partners Community Initiative

Kidney Care Partners recently-launched Performance Excellence and Accountability in Kidney Care (PEAK), a quality improvement campaign with a goal of reducing mortality among first-year dialysis patients by 20 percent by the end of the year 2012. Dr. Jones shares Kidney Care Partners strategy on how they plan to improve survival and help people with kidney disease thrive. About our Guest:  Dr. Edward R. Jones, President of the Renal Physicians Association (RPA) which is also a member of Kidney Care Partners and served as chairman in 2007 and 2008. Dr. Jones is a practicing nephrologist as well as Medical Director for Fresenius Medical Care (Mt Airy Self Care in Philadelphia, Pa). Additionally, Dr. Jones is an attending physician at The Albert Einstein Medical Center (AEMC) and Chestnut Hill Hospital (CHH), and Clinical Professor of Medicine at Temple University Hospital.

Lori Hartwell is the author of Chronically Happy - Joyful Living in Spite of Chronic Illness, and President/Founder of Renal Support Network. A kidney patient since the age of two, she has consistently strived to instill hope into the lives of people who live with chronic kidney disease.
